Section 22 of the ART Act bars any procedure without the written informed consent of all parties. The Rules prescribe the forms. They require ten years of retention. They allow inspection without notice. Most clinics keep all of this on paper in files across cycles.
Four duties. All about records.
- Prescribed forms. The ART Rules 2022 fix the consent forms word for word. Each ends with the clinic's endorsement, a witness from the clinic and the treating doctor's signature.
- Ten-year retention. The Rules require all case records, consent forms and laboratory results to be kept for ten years from completion of the procedure.
- Monthly reporting. A complete report to the National Registry by the 5th of every month.
- No-notice inspection. The place, the equipment and the records may be inspected without any notice.

Before the first cycle
The couple receives Form 12 on WhatsApp in Hindi or their language with audio. They confirm each clause, answer three check questions and sign. The clinic witness and doctor countersign on the dashboard.
Each procedure
Oocyte retrieval, freezing, IUI and donor consents attach to the same couple record. Consent holds across cycles as the Rules intend.
If a couple withdraws
Form 18 is completed on the phone. The original consent is preserved and the withdrawal is sealed alongside it.
